Accidents and dangerous occurrences are required to be reported to the Authority in line with the Safety, Health and Welfare at Work (Reporting of Accidents and Dangerous Occurrences) Regulations 2016 (S.I. No. 370 of 2016). The guidance document below explains why accident and dangerous occurrence reporting is required, what is reportable, what is not reportable, who should make the report and how the report should be made. These are some of the key points in relation to reporting of accidents and dangerous occurrences:

• Only fatal and non-fatal injuries are reportable. Diseases, occupational illnesses or any impairments of mental condition are not reportable.
• Fatal accidents must be reported immediately to the Authority or Gardaí. Subsequently, the formal report should be submitted to the Authority within five working days of the death.
• Non-fatal accidents or dangerous occurrences should be reported to the Authority within ten working days of the event.
• • Injuries to any employee as a result of an accident while at work where the injury results in the employee being unable to carry out their normal work duties for more than three consecutive days, excluding the day of the accident, must be reported to the Authority.

Employers have specific duties to ensure the safety, health and welfare at work of all employees while at the office or at the home office. Under the Display Screen Equipment Regulation 2007 the employer has a number of duties to complly with in it this regulation, the key requirements are to:

• Carry out an analysis or risk assessment of employee workstations.
• Provide information to employees in relation to measures which have been implemented.
• Provide training to employees in the use of workstations before commencing work with display screen equipment and whenever the organisation of the workstation is modified.
• Perform a further analysis or risk assessment where an employee transfers to a new workstation or significant new work equipment, change of equipment or new technology is introduced an an individuals workstation.
• Ensure that the provision of eyesight testing and eye wear is made available to every employee.

• We will contact the employee via video call, we will share our self-assessment webform with the user and our DSE assessor will explain how to complete the self-assessment.
• Following submission of the self-assessment our assessor will review the self-assessment and follow up with a consultation with recommendations on improvements.
